This is a fully remote position, open to applicants in New York.

📋 Description

• Establish and nurture robust and cooperative relationships with clinical partners, which include primary care providers (PCPs), practice managers, and other essential stakeholders across integrated networks.

• Leverage in-person, electronic, and/or telephonic outreach to engage an assigned portfolio of practices, consulting and conveying relevant information succinctly and persuasively to providers and stakeholders.

• Support the onboarding of practices to Honest services and facilitate the effective operationalization of programs while steering your designated practices towards shared objectives and outcomes.

• Educate practices about value-based care opportunities and initiatives that focus on achieving optimal cost efficiencies and patient outcomes.

• Take responsibility for meeting key performance metrics for assigned practices regarding utilization, cost of care, documentation, and quality.

• Employ data to prioritize practice activities and formulate performance strategies that enhance value-based care programs and outcomes.

• Articulate Honest Health programs/services to the partners’ practice engagement teams and coordinate performance strategies across value-based contracts.

• Act as the primary contact and resource for all Honest operational matters, including back-office support, population health tool assistance, and inquiries related to incentives, quality, and CDI.

• Keep precise documentation regarding provider engagement and network initiatives, including contractual documents and Customer Relationship Management (CRM) entries, as necessary.

• Collaborate with Honest Health’s clinical team to support care team objectives aligned with network goals.

• Demonstrate expertise in Honest Health’s business model and discuss insights derived from performance data during provider-facing meetings.

• Work collaboratively across functions internally to pinpoint trends, areas for process enhancement, and opportunities for relationship building.

• Provide reports and updates, as required, to the Director of Market Operations.

• Undertake additional related responsibilities as assigned.


⛳️ Requirements

• A Bachelor's degree in healthcare administration or a related field is preferred; 4+ years of experience in practice management and/or as a physician specialist in a health system or independent physician association (IPA) may be accepted in place of a bachelor’s degree.

• At least 5 years of experience in a healthcare setting, with a proven track record of successfully engaging PCPs, specialists, and health systems; experience with value-based care models is preferred.

• Demonstrated experience as a Practice Manager or in a similar role within healthcare management.

• Strong understanding of clinic operations, medical office management, and experience in clinical supervision and team management.

• Comprehensive knowledge of value-based care key performance metrics and clinical performance management, including coding, quality, and billing.

• Proven capability to analyze and interpret reports to identify practice success opportunities, effectively communicate essential performance components, and translate performance insights into actionable plans for practices.

• Solid grasp of the healthcare delivery system and value-based care principles.

• Comfortable and efficient in a remote work setting, with the ability to travel up to 70% to local provider locations.

• Proven capacity to manage multiple priorities while staying informed about Medicare policies, processes, and procedures.

• Ability to consistently travel to various work sites and maintain a valid driver's license in your state of residence along with motor vehicle insurance.

• Excellent verbal, written, and interpersonal communication skills are essential.

• Demonstrated resilience and adaptability in daily interactions with providers.

• Strong organizational and time management skills.

• Detail-oriented, mission-driven, entrepreneurial, and operates with a sense of urgency.
