Population Health Care Manager – Care Transition

This is a fully remote position, open to applicants in North Carolina.

📋 Description

• Deliver clinical expertise, support, and guidance for patient care transitions upon discharge from the Duke University Health System.

• Provide assistance for inpatient case management, focusing on core administrative functions related to discharge planning.

• Conduct disease and symptom management, assess health conditions, develop care plans, facilitate care, and refer patients to appropriate levels of care.

• Guarantee excellence in care transitions to achieve optimal clinical outcomes through a cohesive model of access and care.

• Enhance the health status and care quality for individuals with chronic conditions, including complex medical, mental health, and psychosocial challenges.

⛳️ Requirements

• Education: A Bachelor's degree in a clinical field such as Nursing, Counseling, Social Work, Therapy, Allied Health, or related community health disciplines.

• Experience: A minimum of 3 years of clinical experience is required.

• Degrees, Licensure, and/or Certification: Must hold a current license in at least one of the following areas: current or compact RN licensure in North Carolina, current licensure as a licensed clinical social worker by the NC Social Work Certification and Licensure Board, current licensure as a Licensed Professional Counselor by the state of NC, or current licensure as a Licensed Addiction Specialist by the state of North Carolina.

• ACM or CCM certification is required within 3 years of the hire date.
