
Population Health Care Manager β Care Transition

This is a fully remote position, open to applicants in North Carolina.
β’ Deliver clinical expertise, support, and guidance for patient care transitions upon discharge from the Duke University Health System.
β’ Provide assistance for inpatient case management, focusing on core administrative functions related to discharge planning.
β’ Conduct disease and symptom management, assess health conditions, develop care plans, facilitate care, and refer patients to appropriate levels of care.
β’ Guarantee excellence in care transitions to achieve optimal clinical outcomes through a cohesive model of access and care.
β’ Enhance the health status and care quality for individuals with chronic conditions, including complex medical, mental health, and psychosocial challenges.
β’ Education: A Bachelor's degree in a clinical field such as Nursing, Counseling, Social Work, Therapy, Allied Health, or related community health disciplines.
β’ Experience: A minimum of 3 years of clinical experience is required.
β’ Degrees, Licensure, and/or Certification: Must hold a current license in at least one of the following areas: current or compact RN licensure in North Carolina, current licensure as a licensed clinical social worker by the NC Social Work Certification and Licensure Board, current licensure as a Licensed Professional Counselor by the state of NC, or current licensure as a Licensed Addiction Specialist by the state of North Carolina.
β’ ACM or CCM certification is required within 3 years of the hire date.
