This is a fully remote position, open to applicants in Ohio.
• Assign appropriate surgical, office procedural, and diagnostic codes (CPT - E/M, surgical, and ICD) to individual patient health records.
• Extract relevant data and address edits within designated time frames.
• Review and assess patient medical records to ascertain the level of Evaluation and Management (E/M) service, identify non-E/M office procedures, surgical and interventional procedures, as well as diagnoses.
• Precisely assign and sequence CPT codes, modifiers, and ICD codes.
• Consult with physicians when code assignments are complex or when documentation in the record is insufficient, ambiguous, or unclear for coding purposes.
• High School diploma or equivalent is mandatory.
• Certification in coding (CPC, COC, CCS, CCS-P, RHIA, RHIT) is required.
• A minimum of one year of coding experience in a physician office is necessary.
• Formal training in CPT and ICD coding or relevant work experience applying ICD and CPT coding principles is essential.
• In-depth knowledge of procedure and diagnostic coding for professional services, along with Medicare, Medicaid, and other third-party payer coding and billing regulations.
• Proven understanding of Evaluation and Management Documentation Guidelines and other professional documentation requirements.
