Physician Assistant

This is a fully remote position, open to applicants in Louisiana.

📋 Description

• Oversee a panel of families as the main healthcare provider, enhancing overall family health through continuous primary and preventive care, care management, and coordination in both home and virtual settings (including video, phone, and SMS).

• Collaborate with a comprehensive interdisciplinary team, comprised of MDs, LCSWs, RNs, psychiatrists, dieticians, and medical assistants.

• Leverage Nest Health’s electronic health records and care management tools to gather data, document member interactions in the field, organize information, track tasks, and communicate effectively with your team, members, and community resources.

• Attend daily team huddles and weekly case conferences to provide insights on physical health needs and set health goals as part of each member’s Nest Action Plan (NAP).

• Recommend treatments for chronic and infectious diseases, prescribe and manage medications, and educate patients regarding their conditions and prescribed treatments.

• Order, administer, collect, and document diagnostic tests.

• Offer information and assistance through advocacy, navigation support, and education to Nest families on the availability and eligibility of health and social care services.

• Engage in outreach initiatives to Nest families, including explaining program details to prospective families.

⛳️ Requirements

• Completion of an accredited Physician Assistant program and possession of an active, unrestricted license to practice is mandatory.

• Minimum of 2 years of experience providing clinical services to pediatric (including newborns) and adult patients with both chronic medical and behavioral health conditions, along with a strong interest in serving underserved, vulnerable populations.

• Proven proficiency, previous experience, and/or a willingness to train in clinical nursing skills such as blood drawing (venipuncture & phlebotomy), conducting patient assessments, and reinforcing care plans for common adult and pediatric chronic conditions like diabetes, respiratory issues, hypertension, CHF, depression, etc.

• Demonstrated skills, knowledge, and judgment necessary for independent clinical decision-making and problem-solving.

• Availability to work full-time for a five-day work week and participate in a rotating call schedule that may include occasional evenings and weekends each month.

• Must maintain current CPR certification (BLS).

• Experience as a leader of an interdisciplinary care team is preferred.

• Familiarity with a startup environment is beneficial, though not required.
