Physical Therapist – Home Health

This is a fully remote position, open to applicants in Florida.

📋 Description

• Conduct initial and ongoing assessments of patients, including OASIS evaluation and documentation.

• Create and implement tailored treatment plans in collaboration with physicians and healthcare professionals.

• Modify treatment plans as necessary.

• Work alongside interdisciplinary care teams to enhance patient outcomes.

• Provide education and support to patients, families, and caregivers regarding therapy programs.

• Keep precise clinical records and inform physicians of any changes in patient conditions.

• Oversee Physical Therapy Assistants in accordance with agency policies and state regulations.

• Engage in in-services, case conferences, and team meetings.

• Assess needs for additional services and make suitable referrals.

• Confirm daily visit schedules.

• Relay patient progress and changes to physicians and care teams.

• Document all visits and progress notes on the same day of service.

⛳️ Requirements

• Active state-specific PT license.

• Graduate from an accredited physical therapy program.

• Valid BLS certification.

• At least one year of experience in physical therapy; home health experience is preferred.

• Strong communication, organizational, and time management abilities.

• Valid driver’s license and access to a reliable, insured vehicle.

• Capability to visit homes that may not be wheelchair accessible.

• Adequate hearing, vision, and dexterity to conduct physical assessments and demonstrate care techniques.

• Ability to walk, stand, bend, and occasionally lift or reposition patients.

• Familiarity with electronic medical records is an advantage.
