This is a fully remote position, open to applicants in California.
• Assess patients’ functional abilities, mobility, muscle strength, gait, and range of motion to create personalized treatment plans.
• Provide physical therapy services in alignment with the physician's care plan and according to California’s Physical Therapy Practice Act.
• Inform patients and their families or caregivers about therapeutic exercises, equipment usage, and recovery objectives.
• Collaborate with physicians and the healthcare team, communicate changes in patient status, and engage in case discussions.
• Supervise Physical Therapy Assistants and oversee Home Health Aide services bi-weekly when PT is the only skilled service provided.
• Handle documentation tasks, including clinical progress reports and discharge planning.
• Identify the need for additional home health services and coordinate appropriate referrals.
• Carry out other responsibilities as necessary for patient care.
• Bachelor’s or Master’s degree in Physical Therapy from an accredited institution.
• Active Physical Therapy license in California.
• A minimum of 2 years of experience in physical therapy; experience in home health is an advantage.
• Excellent communication skills and the capacity to establish rapport with patients and their families.
• Current CPR certification from the American Heart Association.
• Valid driver's license and a reliable, insured vehicle.
