
Pharmacy Care Coordinator, MA – Contract to Perm

This is a fully remote position, open to applicants in United States.
• Perform extensive outbound telephone outreach to patients to present Evergreen's pharmacy service, build rapport and trust, and assist patients with the enrollment process.
• Provide clear, standardized messaging by utilizing approved scripts and talking points; address patient inquiries and concerns.
• Secure and document patient consent via phone, as well as collect enrollment details.
• Update patients' preferred pharmacy information in the EHR, initiate electronic prescriptions, and record consent or refusal.
• Oversee outreach queues and follow-up schedules, including multiple contact attempts and prioritization based on urgency and targets.
• Record call activity, outreach attempts, acceptance/declination, and coordination efforts in clinical and operational systems.
• Achieve call center performance benchmarks related to call quality, productivity, average speed of answer, and enrollment/conversion goals.
• Adhere to patient privacy and confidentiality guidelines in line with HIPAA and organizational policies.
• Execute additional duties related to the role as assigned.
• Valid Medical Assistant certification (CMA/RMA).
• Experience in a telephonic, call-center, or high-volume outbound patient/customer outreach setting.
• Excellent interpersonal and communication skills, with the capability to build trust and engage patients over the phone.
• Background in healthcare, pharmacy, care coordination, or other patient support roles.
• Proficient and comfortable using an electronic health record (EHR/EMR), including documenting encounters, reviewing medication lists, and making authorized updates.
• Strong organizational abilities and meticulous attention to detail, with the capacity to manage multiple patients, tasks, and follow-ups within an outreach queue.
• Familiarity with HIPAA regulations and a commitment to safeguarding patient privacy and PHI.
• Intermediate proficiency in MS Office Suite, including Outlook and Teams.
• Home internet connection that supports a minimum download speed of 25 Mbps and upload speed of 10 Mbps.
• Ability to work from a dedicated space free from ongoing interruptions to protect member PHI/HIPAA information.
• Preferred: previous experience in a healthcare call center or telephonic patient outreach/enrollment.
• Preferred: bilingual in English and Spanish.
• Preferred: experience with phone-based consent capture or enrollment workflows.
• Preferred: background in care navigation, including connecting patients to community resources and addressing social determinants of health.
• Preferred: knowledge of common chronic disease medications and terminology related to prescription coverage.
