This is a fully remote position, open to applicants in New Hampshire.
• Review patient-care notes and assessments from clinicians to gather prior authorization data and care-plan recommendations.
• Create and enhance clinical protocols specific to diseases and medications.
• Oversee patient adherence across various sites and specialty disease categories, with a focus on oncology.
• Assess EMR data, including drug and disease interactions, medication selection, lab results, allergies, prescriptions, renewals, and related clinical information.
• Suggest therapeutic regimens and disease-specific treatments based on established guidelines.
• Track therapies for side effects and medication-related concerns, recommending modifications to prescribers as needed.
• Conduct medication therapy management, documenting interventions and evaluating adherence rates.
• Facilitate pharmacist consultations, motivational interviewing/coaching, medication reconciliation, regimen optimization, synchronization, reminders, and care coordination.
• Monitor productivity and provide weekly growth reports.
• Supervise daily activities of technicians and ensure accuracy in their verifications.
• Identify and address insurance issues and opportunities for financial assistance.
• Manage communications among support personnel in accordance with HIPAA regulations.
• Ensure compliance with applicable pharmacy laws and DEA requirements while maintaining pharmacist registration and remote pharmacist activities.
• Engage directly with other healthcare professionals regarding patient care.
• Train pharmacy staff, students, interns, externs, and other healthcare professionals.
• Manage and update relevant information using database applications.
• Support initiatives for operational improvements, productivity, quality, and customer service standards.
• Assist with research projects and peer review activities.
• Identify trends, troubleshoot issues, suggest enhancements, and implement changes.
• Participate in initiatives aimed at process improvement.
• Safeguard confidential organizational information.
• Maintain professional knowledge through ongoing education, publications, networking, and involvement in professional organizations.
• Perform additional duties as assigned.
• A Doctor of Pharmacy (PharmD) or an equivalent advanced degree is required.
• A PharmD or an equivalent advanced degree is mandatory.
• Must possess a valid Pharmacist license.
• NH pharmacist licensure is required based on the role title/Job Location metadata.
• Current registration must be maintained in compliance with local, state, and federal pharmacy laws and DEA regulations.
• A minimum of 1 year of experience is strongly preferred but not mandatory.
• Extensive knowledge of specialty medication pharmacology is essential.
• Familiarity with local, state, and federal pharmacy regulations is required.
• Strong technical and clinical competencies are necessary.
• Ability to analyze information and make sound judgments is important.
• Excellent verbal and written communication skills are a must.
• Capability to manage processes and personnel effectively.
• Proficiency in Microsoft Word, Excel, and PowerPoint is necessary.
• Computer literacy and a working knowledge of database and reporting tools are required.
• Solid computer skills, including proficiency in Outlook, Teams, Word, Excel, PowerPoint, and web-based platforms, are essential.
• Data-entry and computer skills will be assessed during the pre-employment evaluation.
• Ability to identify barriers to medication adherence and develop and monitor improvement plans is crucial.
• Capacity to motivate and coach patients towards improved outcomes and therapy goals is important.
• Preferred knowledge of complex/chronic disease states and specialty-pharmacy therapies, including oncology, transplant, hepatitis C, infectious disease/HIV, multiple sclerosis, rheumatoid arthritis, GI/IBD, cystic fibrosis, psoriasis, hyperlipidemia, lupus, and other inflammatory conditions.
• Must be organized, high-integrity, detail-oriented, dependable, quality-focused, empathetic, and an effective listener and communicator.
• Should be an energetic, highly motivated team player with strong interpersonal and communication skills.
• Must exercise discretion and maintain confidentiality when handling sensitive and private information.
• Familiarity with EPIC/QS1/WILLOW is advantageous but not required.
