Pharmacist

This is a fully remote position, open to applicants in Hawaii.

📋 Description

• Deliver extensive Medication Therapy Management (MTM) services

• Perform medication reconciliation and evaluate the effectiveness of therapies

• Provide patient-centered education concerning medications and chronic illnesses

• Assist in managing chronic diseases (e.g., diabetes, hypertension, COPD, CHF)

• Employ motivational interviewing strategies to enhance adherence and participation

• Work collaboratively with healthcare providers to refine medication regimens and care strategies

• Detect and address medication-related challenges and potential risks

• Accurately document patient interactions in line with program standards

• Facilitate care transitions and initiatives for continuity of care

⛳️ Requirements

• Doctor of Pharmacy (PharmD)

• Valid pharmacist license in Hawaii (HI)

• Openness to obtaining additional state licensure if necessary

• Proficiency in Medication Therapy Management (MTM)

• Background in ambulatory care or outpatient environments

• Experience with chronic disease management

• Knowledge in patient education and engagement

• Familiarity with care programs like CCM, TCM, and RPM is advantageous
