This is a fully remote position, open to applicants in United States.
• Confirm interactions, dosing, or contraindication findings against the comprehensive medication list and chart of the patient.
• Identify overlooked interactions, outdated patient data, erroneous dose calculations, and obsolete monograph guidance.
• Assess whether all active medications, current laboratory results, comorbidities, and the overall clinical context have been evaluated.
• Differentiate between allowed treatments and those suitable for the particular patient.
• Ensure that clinical conclusions are accurately reflected in order updates, prescriber notifications, and chart/note documentation.
• Participate in projects aimed at training and improving AI systems by leveraging pharmacy expertise.
• Regular use of UpToDate Lexidrug (or Lexicomp) at least weekly in a current or recent position.
• Minimum of 2 years of professional pharmacy experience.
• Experience in an inpatient/hospital environment is highly preferred.
• Capability to perform thorough medication-list interaction checks, including assessing severity ratings and interaction mechanisms.
• Proficiency in evaluating contraindications, boxed warnings, and other warnings/precautions in relation to specific patients.
• Ability to differentiate between “not contraindicated” and “appropriate for this patient.”
• Competence in reconciling Special Alerts with monograph content.
• Skill in applying renal/hepatic dose-adjustment logic and calculators utilizing current patient data.
• Strong knowledge in at least 2 to 3 specialized areas: Trissel's IV Compatibility; pediatric/weight-based dosing; Beers Criteria/geriatric precautions; pregnancy and lactation risk categories; REMS and prescribing/access restrictions; therapeutic class comparisons; drug identification and patient education/multi-language handouts.
• PharmD and an active pharmacy license, or equivalent extensive clinical experience with verification.
• Comfort in ensuring that clinical findings are documented and communicated through the chart, order, and care team interactions.
• Previous experience in data annotation, labeling, or AI evaluation is advantageous.
• Must not qualify as an H1-B or STEM OPT candidate.
