This is a fully remote position, open to applicants in New York.
• Administering intravenous medications, including chemotherapy, IVIG, and monoclonal antibodies.
• Conduct start of care and follow-up visits.
• Act as a liaison among pharmacy, CHHA, and the care team to ensure coordination of all patient care aspects.
• Deliver skilled infusion therapy within the home environment.
• Manage vascular access devices, such as peripheral IVs and central venous catheters (CVCs) including PICC lines, MediPorts, Hickman, and Broviac catheters, providing insertion (peripheral IVs), maintenance, medication administration, dressing changes, flushing, and monitoring for any complications.
• Educate patients and their families regarding infusion therapy and CVC management.
• Perform ongoing patient assessments, including dressing changes, phlebotomy, and care management.
• Communicate any changes in patient condition promptly and effectively to physicians and interdisciplinary care teams.
• Maintain accurate, timely, and comprehensive clinical documentation in accordance with agency policies and regulatory standards.
• Ensure strict compliance with HIPAA regulations while upholding patient privacy and confidentiality standards.
• A minimum of two (2) years of infusion nursing experience in an acute care hospital setting or a comparable clinical environment.
• Current IV Therapy Certification.
• Bilingual proficiency is highly preferred (Spanish, Russian, Mandarin, etc.).
• Current Basic Life Support (BLS) certification.
• Advanced Cardiovascular Life Support (ACLS) certification is strongly preferred.
• Proven knowledge of federal HIPAA regulations and a commitment to maintaining patient privacy and confidentiality.
• Ability to accurately interpret and execute physician orders, treatment plans, and other clinical documentation.
• Strong capability to collaborate effectively as part of an interdisciplinary healthcare team.
• Excellent communication, organizational, and critical-thinking skills, with the ability to provide safe, patient-centered care.
• Competitive hourly compensation ($55-$70).
