This is a fully remote position, open to applicants in North Carolina.

📋 Description

• Execute in-reach, engagement, educational, and support initiatives for adults experiencing serious mental illness or persistent mental health challenges.

• Provide members/recipients with information regarding community-based mental health services and options for supported housing, including tenancy support and rental assistance.

• Educate members/recipients about Medicaid, Special Assistance, North Carolina State Plan services, and state-funded programs.

• Fulfill daily and monthly minimum requirements for in-reach, transition, diversion, and informed decision-making activities.

• Maintain the in-reach caseload, ensuring that each individual receives a visit from a Peer Support Specialist within 90 days.

• Organize face-to-face meetings with members/recipients and their guardians concerning integrated supported housing.

• Evaluate interest in supported housing and articulate its benefits and financial implications.

• Assist members/recipients by facilitating and accompanying them on site visits to permanent housing options.

• Address and explore concerns of members/recipients who may decline or feel uncertain about supported housing.

• Review housing preferences and connect individuals to ongoing in-reach and available housing resources.

• Keep detailed records of all activities and monitor in-reach outcomes, refusals, and unresponsiveness.

• Participate in state-level training, information sessions, and committees associated with the Transitions to Community Living Initiative in North Carolina.

• Collaborate with Transition Coordinators, care management teams, and other departments within Vaya to facilitate smooth transitions and resolve barriers.

• Carry out additional tasks as assigned.


⛳️ Requirements

• A high school diploma or GED is mandatory.

• 1–2 years of experience working with the mental health/substance use population is preferred.

• Certification as a NC Certified Peer Support Specialist is required.

• Familiarity with recovery methods and person-centered approaches is essential.

• Strong customer service skills are necessary.

• Capability to build respectful relationships with individuals with disabilities and their families is vital.

• Ability to interact with professionalism and effectiveness, particularly in challenging situations.

• Excellent trust-building, rapport, and communication skills are important.

• Understanding of human services laws, regulations, and policies is advantageous.

• Knowledge of Medicaid and Special Assistance benefits, clinical services, community supports, and supported housing is beneficial.

• Ability to articulate ideas clearly and succinctly, both orally and in writing, is required.

• Capacity to work efficiently as a team member and under the guidance of the Peer Support Specialist Manager is essential.

• Must reside in North Carolina or within 40 miles of the North Carolina state line.

• Adherence to confidentiality and protected health information regulations, including HIPAA and 42 CFR Part 2, is necessary.

• Completion of required confidentiality training and signing a confidentiality agreement is obligatory.

• Must be able to meet the visual, physical, sedentary, lifting, and concentration requirements as stated.
