This is a fully remote position, open to applicants in Washington.
• Deliver comprehensive medication management services that encompass medication access, education, adherence, directed medication management, and support for quality metrics within the assigned region, in accordance with the frameworks established by the medication management center of excellence.
• Facilitate medication access management services by coordinating between patients, pharmacies, insurance providers, and healthcare professionals, while overseeing and supporting the regional Medication Support Navigator.
• Offer precise and comprehensive medication education, adherence assessments, and directed medication management through direct engagement with patients and caregivers, as well as collaborating with both internal and external care teams, while working independently within the defined scope of clinical practice (when applicable).
• Record interactions in the patient's electronic medical record, ensuring effective communication with the patient care team.
• Advocate for evidence-based and cost-efficient medication management by staying informed about new medications, devices, and relevant medical literature.
• Document and report adverse drug events (ADEs), medication allergies, and medication errors in accordance with established procedures.
• Act as a liaison between the assigned region and the medication management center of excellence.
• Adhere to all regulations and standards set by governing authorities in the pharmacy practice.
• Perform additional responsibilities as required.
• Must be a graduate of an accredited pharmacy college/university (Pharm.D) required.
• Must possess an unrestricted and current pharmacist license with the capability of obtaining licensure by reciprocity in each state where telehealth services will be delivered.
• Active license in Washington is strongly preferred.
• Completion of an ASHP accredited PGY-1 Pharmacy Residency is mandatory.
• Completion of an ASHP accredited PGY-2 Pediatric or Ambulatory Care Pharmacy Residency is preferred.
• Board Certification as a Pediatric Pharmacy Specialist (BCPPS), Pharmacotherapy Specialist (BCPS), or Board-Certified Ambulatory Care Pharmacist (BCACP) is preferred.
• Candidates without board certification must be eligible for board certification in a preferred specialty area and are expected to pursue and successfully obtain certification within an agreed timeframe.
• Bilingual proficiency in English and Spanish is preferred.
• A minimum of 2 years of practice experience in pediatrics, post-PGY1 residency.
• Experience in a pediatric or ambulatory care setting is strongly preferred.
