📋 Description

• Significantly enhances the effectiveness of integrated primary care by ensuring high-quality patient care through preventive and wellness initiatives.

• Aids a clinic by overseeing and coordinating care for a designated panel of patients.

• Monitors patient attribution, identifies care gaps, coordinates preventive services, and reaches out to patients or healthcare providers to facilitate care coordination.

• Ensures that patients' needs are addressed by connecting them with appropriate and accessible services, care team members, and resources to promote ideal health and wellness.

• Assists the clinic care team in managing and coordinating preventive screenings and wellness programs for optimal health and wellbeing.

• Provides oversight for providers' patient panels by tracking patient attribution to maintain the integrity of the EMR, organizing wellness screenings for specific specialties, supporting office visits, procedures, and scheduling screenings as necessary, and managing chronic disease registries, provider scorecards, and result reconciliation.

• Advocates for quality initiatives and enhanced patient outcomes.

• Engages in the care of age-related patients across all stages of preventive care and health maintenance.

• Operates within the administration's established scope of practice guidelines.

• Tracks and reports quality measures while validating the accuracy and integrity of patient records.

• Participates in patient care by gathering subjective and objective data from the patient or caregiver to identify potential barriers to care (SDoH) and refers to care management roles as necessary.

• Communicates the collected data and secures necessary orders to support the healthcare team, following through on the patient's care plan under the direction of the care team.

• Exhibits proficiency in computer skills, including the use of Microsoft Office applications and electronic health records (EHR).


⛳️ Requirements

• Post-secondary education of one to two years, preferably with a focus on healthcare or a medical-related field.

• At least one year of work experience in ambulatory care, community outreach, patient or provider support, customer service, and/or various integrated care systems is preferred.

• Training in Motivational Interviewing and/or Mental Health First Aid is preferred.

• A background as a Certified Medical Assistant (CMA) is required.
