Patient Engagement Specialist

This is a fully remote position, open to applicants in Florida.

📋 Description

• Serve as an essential member of the Heart Failure Program.

• Collaborate closely with the heart failure clinical team to ensure patient engagement, success, and satisfaction.

• Oversee and manage a list of patients eligible for enrollment.

• Conduct outreach to a targeted list of patients promptly while maintaining proper documentation.

• Strive to enhance patient engagement by professionally informing patients and/or caregivers about the personalized care and services available to them.

• Assist patients in scheduling appointments in alignment with the Heart Failure program.

• Manage and coordinate inbound and outbound calls in a professional, courteous, and timely manner.

• Screen calls based on the caller's needs and assist in resolving or redirecting the call as necessary.

• Oversee the tracking and coordination of mailings in partnership with the care team.

• Ensure that patient demographics in the medical record are updated with current information.

• Review and address voicemail messages promptly.

• Strive to consistently deliver an exceptional patient experience.

• Build strong rapport and maintain long-term relationships with patients and caregivers.

• Participate in staff meetings.

• Perform other duties as assigned.


⛳️ Requirements

• High school diploma or GED.

• At least one year of experience in customer service or a healthcare call center.

• Previous experience in a medical office setting or other healthcare experience is advantageous.

• Familiarity with telehealth is a plus.

• Strong interpersonal skills for effective and pleasant communication with patients and other care team members.

• Excellent customer service skills aimed at achieving high levels of customer satisfaction.

• Proficient in taking initiative, multi-tasking, and organizing tasks.

• Possess a positive attitude, compassion, and a preference for working in a team-oriented environment.

• Experience with electronic health records, such as Athena, as well as proficiency in Microsoft Word, Excel, and other computer applications is highly preferred.

• Bilingual fluency in English and Spanish is required.
