This is a fully remote position, open to applicants in Philippines.
• Act as a bridge between contracts, patients, their families, Home Health Aides (HHAs), and the care team.
• Ensure seamless communication and pinpoint areas of concern during interactions.
• Assist in delivering exceptional patient care.
• Organize work assignments based on Home Care Worker training.
• Accurately document all communications in HHA Exchange.
• Effectively engage with patients, family members, clinicians, and various departments.
• Provide relevant information to healthcare professionals.
• Consult with family members to guarantee that ongoing care requirements are fulfilled.
• Offer guidance regarding discharges and modifications in services.
• Complete necessary reports accurately and in a timely manner.
• Oversee requests for overtime and travel time.
• Use tools to locate aides and provide them with directions.
• Manage mutual and linked cases while promptly responding to emails and voicemails.
• Quickly adapt to changes within the agency or industry.
• Outstanding customer service and active listening abilities.
• Capability to multitask efficiently.
• Strong written and verbal communication skills.
• Effective time management skills and ability to prioritize tasks.
