This is a fully remote position, open to applicants in United States.
β’ Act as a point of contact for members following their intake appointment, as requested or accepted via the platform.
β’ Guide members on how to access healthcare services, including locating providers, utilizing insurance information, and scheduling appointments.
β’ Assist members with navigation challenges, such as online portals, referrals, appointment logistics, application forms, referral workflows, or communications with healthcare providers.
β’ Leverage information provided through the platform, including details related to Social Determinants of Health, to identify practical support needs and available resources.
β’ Collaborate, when appropriate, with clinical professionals involved in a member's care while offering only non-clinical navigation support.
β’ Record member interactions and support activities through the platform in line with applicable platform requirements.
β’ Deliver member support in a warm, empathetic, and culturally responsive manner.
β’ Utilize independent judgment to determine the best way to provide services, aligning with member needs, applicable laws, and platform standards.
β’ A minimum of 3 years of experience in providing member-facing support within healthcare, social services, benefits navigation, care coordination, member advocacy, or a related field.
β’ Strong understanding of the U.S. healthcare system, including Medicare and common navigation challenges faced by Medicare patients.
β’ Ability to work independently and apply sound judgment when addressing member needs.
β’ Capability to manage member support activities autonomously, including prioritizing tasks, following up on member requests, and maintaining proper documentation.
β’ Proficiency in using technology platforms, which may include EHRs, CRMs, member messaging systems, scheduling applications, or other digital health tools.
β’ Experience in supporting older adults, individuals with complex healthcare needs, or other groups that may encounter barriers to accessing care.
β’ Relevant professional credentials, such as RN, CMA, BCPA, or similar certification, are preferred but not mandatory.
