This is a fully remote position, open to applicants in United States.
• Deliver exceptional preventative, acute, and chronic care in a fully-remote, virtual environment.
• Enhance remote healthcare by offering compassionate and impactful patient interactions.
• Oversee a dedicated panel of patients as their primary care physician.
• Refer patients to specialists or acute care services when clinically necessary.
• Engage in population health initiatives for risk-attributed patients (Annual Wellness Visits, addressing care gaps, transitions of care, risk capture).
• Assist patients immediately following discharge to enhance their understanding of treatment plans and minimize hospital readmissions.
• Continuously assess personal performance to ensure compliance with clinical guidelines and best practices.
• Over 3 years of post-residency experience.
• At least 2 years of experience in a traditional primary care environment.
• Required experience with virtual consultations.
• Possess an active IMLC compact license or reside in an IMLC member state.
• Previous experience with EPIC EMR is essential.
• Must have a clean background and medical malpractice history.
• Willingness to commit to 24 hours per week.
• Flexibility to accommodate a varied schedule, which may include specific weekdays, evenings, and weekends.
• Proficiency in Spanish is a plus.
• Strong desire to build long-term relationships with patients and manage chronic care conditions.
• Comfort in managing acute conditions in a virtual setting.
• Extensive experience handling 3 or more cases per hour.
