Part Time Licensed Practical Nurse, CA License Required

This is a fully remote position, open to applicants in United States.

📋 Description

• Perform outbound telephonic outreach to patients and offer individualized coaching and support for those managing chronic conditions, including but not limited to type 2 diabetes, hypertension, and cardiovascular disease.

• Assist patients in implementing customized care plans and reaching their objectives by delivering education and coaching centered on behavior modification, nutrition, physical activity, and self-management strategies utilizing evidence-based techniques such as motivational interviewing and SMART goals.

• Carry out thorough assessments of patients' health status, lifestyle behaviors, nutritional practices, and their readiness to make changes.

• Provide education on disease management, medication adherence, symptom recognition, and prevention strategies, while performing care delivery tasks in alignment with Cadence clinical policies and established protocols, under suitable clinical supervision.

• Track patients' progress, compliance with treatment plans, and health outcomes through routine check-ins and remote monitoring tools.

• Record patient interactions, outcomes, and follow-up plans in the Cadence platform; identify barriers to care (including social factors) and connect patients with resources or internal support systems.

• Collaborate in real-time with clinicians and patient support teams using platforms such as our Cadence Platform, Google Workspace, and Slack, while providing feedback to enhance workflows in a dynamic, change-oriented setting.

⛳️ Requirements

• An active compact multi-state LPN/LVN license in the state of residence is mandatory.

• A minimum of 5 years of clinical experience as a Licensed Practical Nurse is required.

• Readiness for remote work (including stable high-speed WiFi) is essential.

• An active LVN license in California is required.

• Proficiency in Spanish is preferred.

• Experience in providing patient education and support to individuals managing chronic conditions is advantageous.

• Familiarity with AI-assisted clinical tools (such as Gemini, Open Evidence, or similar platforms) and the capability to engage credibly with providers and patients regarding the role of AI in care delivery and the aspects where human judgment is essential.
