Oncology Survivorship Advanced Practice Provider

This is a fully remote position, open to applicants in United States.

📋 Description

• Perform comprehensive evaluations of cancer survivors through telemedicine to ascertain their physical, emotional, and psychosocial requirements.

• Offer evidence-based recommendations and educational resources, working in close partnership with the survivorship care team to recognize and meet patients’ needs, taking into account their personal history and distinct experiences.

• Formulate and revise survivorship care plans in collaboration with patients, integrating insights from behavioral health professionals to promote holistic well-being.

• Effectively collaborate and communicate with all members of the patient care team, both internal and external, including oncologists, primary care providers, and other specialists.

• Educate and support patients about cancer survivorship, ongoing care requirements, and available resources, utilizing OncoveryCare’s evidence-based educational materials.

• Partner with clinical leadership to develop and enhance survivorship care programs, incorporating patient feedback and best practices to elevate patient experiences and outcomes.


⛳️ Requirements

• 3-5+ years of advanced practice experience within an oncology healthcare environment.

• Ideally experienced in delivering cancer survivorship care or possessing substantial symptom/supportive care management expertise.

• Master’s degree in a relevant field.

• Current certification as a Nurse Practitioner or Physician Assistant in a suitable specialty for adult patient care.

• Openness to obtaining licensure as an Advanced Practice Provider in designated states as necessary.

• Dedication to self-directed learning to remain current with clinical knowledge and evolving evidence-based practices in healthcare.

• Capability to convey complex medical information to patients in an easily understandable manner, adjusting to individual patient requirements.

• Exceptional communication, empathy, and active listening abilities.

• Proficiency in utilizing and documenting within electronic medical records in an effective and timely manner.

• Ability to independently advance work initiatives while collaborating efficiently with diverse stakeholders.

• Full-time availability for a 40-hour workweek, Monday to Friday, from 9 am to 5 pm.
