OCM RN Case Manager

This is a fully remote position, open to applicants in California.

📋 Description

• Accountable for the clinical care and coordination of services rendered to patients in their home or residential setting.

• Delivering clinical services, including comprehensive assessments, medication administration and management, as well as executing treatments prescribed by the physician.

• Identifying, organizing, coordinating, and overseeing the services required by a recipient.

• Utilizing the nursing process to evaluate and address the patient's medical needs.

• Providing nursing services, as a member of the Case Management Team, for each assigned caseload and client.

• Ensuring accurate tracking, documentation, progress notes, and case records for enrolled clients.

• Recording patient interventions and responses for precise documentation.

• Guaranteeing proper timekeeping and scheduling as discussed with the supervisor.

• Collaborating effectively with the Social Worker on their Case Management Team.

⛳️ Requirements

• Must hold a current, unsuspended, un-revoked license to practice as a Registered Nurse in California.

• Familiarity with the current Nursing Practice Act.

• Holds current CPR certification.

• Skilled in computer use.

• A minimum of two (2) years of experience in a home health organization, primary care clinic, or healthcare facility.

• One (1) year of experience in healthcare quality management is preferred.

• Current and satisfactory results from a pre-employment physical examination, including TB Screening Test or chest X-ray.

• Must be free from any signs of infection or illness.

• Demonstrates excellent judgment and possesses knowledge of current nursing practices, with proven exemplary clinical, analytical, and documentation skills.

• Exhibits strong written and verbal communication abilities.

• Fluent in English.

• Must be a licensed driver with an insured automobile that is in good working condition, in accordance with state and/or organizational requirements.
