Nutritional Support Coordinator

This is a fully remote position, open to applicants in Alabama, +13 more states.

📋 Description

• Patient Outreach & Nutritional Counseling: Oversee a designated group of 300-350 patients while conducting necessary monthly nutritional outreach and support. Make 50-60+ daily phone calls to patients, offering personalized nutritional care plan assistance and health coaching tailored to their specific needs. Uphold call quality standards and maintain professional communication in every patient interaction; participate in regular QA call reviews.

• Nutrition Planning & Patient Education: Provide education to patients on nutritional principles, dietary adjustments, and weight management techniques. Break down complex nutritional information for various patient demographics. Reinforce provider-directed nutritional recommendations and encourage patient adherence to established care plans.

• Care Coordination & Collaboration: Work in partnership with clinical teams, providers, and caregivers to identify and meet nutritional and clinical needs. Support care coordination objectives by adhering to monthly outreach targets. Escalate patient concerns or changes in health status to the relevant clinical staff following established protocols.

• Documentation & Compliance: Record all patient interactions in real time using the care coordination platform. Ensure HIPAA compliance in all patient communications and documentation. Maintain precise documentation to support care coordination objectives.

⛳️ Requirements

• A minimum of 2+ years of experience in a healthcare position such as care coordination, health coaching, or as a Medical Assistant.

• Must possess a nationally recognized medical assistant certification (CMA, RMA, CCMA, NCMA, CMAC, or an equivalent state-issued MA-C).

• OR an active Licensed Practical Nurse or Licensed Vocational Nurse license: Must hold a multi-state compact license.

• At least 2+ years of experience in clinical nutrition counseling or dietary care coordination.

• 2+ years of experience with Electronic Medical Records (EMR) and navigating computer systems.

• Proven ability to educate patients and simplify complex nutritional concepts.

• HIPAA-compliant home office setup with a minimum of 50 Mbps download and 5 Mbps upload internet capability.

• Must be legally authorized to work in the United States.
