Nurse Practitioner Team Leader

This is a fully remote position, open to applicants in Alaska, +9 more states.

📋 Description

• Guide, mentor, and cultivate a team of 10-12 Continuity Clinicians to meet clinical, operational, and engagement targets.

• Foster an environment centered on accountability, continuous enhancement, and exceptional performance.

• Hold regular one-on-one meetings, performance assessments, career development discussions, and ongoing coaching sessions.

• Detect performance deficiencies and proactively create improvement strategies to facilitate clinician success in collaboration with the Clinical Education Lead.

• Acknowledge and nurture high-achieving clinicians, providing avenues for growth and leadership advancement.

• Track team performance across quality, clinical outcomes, member satisfaction, and productivity metrics.

• Conduct periodic evaluations of clinician performance and deliver constructive feedback to enhance care delivery.

• Engage in clinical quality assurance activities and support compliance with evidence-based care standards.

• Recognize trends, challenges, and opportunities that affect team performance and patient outcomes.

⛳️ Requirements

• NP degree with at least one active state medical license.

• A minimum of 2 years of direct management experience involving coaching, performance management, and employee development.

• Proven track record of leading teams to accomplish clinical, quality, productivity, and engagement objectives.

• Experience in analyzing performance data and leveraging insights to promote accountability and continuous improvement.

• Strong coaching and feedback capabilities, with a genuine passion for developing others.

• Proficient in identifying best practices, resolving issues, and facilitating the adoption of changes within a clinical context.

• Excellent communication, collaboration, and relationship-building abilities.

• Comfortable managing a patient panel including individuals with diabetes, pre-diabetes, and obesity.
