
Nurse Practitioner – In Home Family Care

This is a fully remote position, open to applicants in Louisiana.
• Oversee a group of families as the primary healthcare provider to enhance overall family health through continuous primary and preventive care, care management, and coordination within home settings and through telehealth and virtual care options such as video, phone, and SMS.
• Collaborate with a strong interdisciplinary team, which includes MDs, LCSWs, RNs, psychiatrists, dieticians, and medical assistants.
• Leverage Nest Health’s electronic health record and care management systems to gather data, document member interactions in the field, organize information, monitor tasks, and communicate effectively with your team, members, and community resources.
• Participate in daily team huddles and weekly case conferences to provide insights on physical health requirements and to set health objectives as part of the member’s NAP (Nest Action Plan).
• Recommend treatments for chronic and infectious diseases, prescribe and manage medications accordingly, and educate patients regarding their health conditions and medications.
• Order, administer, obtain, and document diagnostic tests.
• Offer information and assistance through advocacy, navigation support, and education to Nest families concerning the availability and eligibility of health and social care services.
• Engage in outreach initiatives to Nest families, which includes clarifying program details to prospective families.
• Completion of an accredited Physician Assistant or Nurse Practitioner program and possession of an active, unrestricted license to practice in Louisiana is mandatory.
• A minimum of 2 years of experience delivering clinical services to pediatric (including newborns) and adult patients with both chronic medical and behavioral health challenges, along with a desire to serve underserved and vulnerable populations.
• Demonstrated expertise, prior experience, and/or a willingness to learn clinical nursing skills such as blood drawing (venipuncture & phlebotomy), patient assessments, and care plan reinforcement for common adult and pediatric chronic conditions like diabetes, respiratory issues, hypertension, CHF, and depression.
• Proven competencies, knowledge, and judgment essential for independent clinical decision-making and problem-solving.
• Availability to work full-time, five days a week, and participate in a rotating call schedule that may include occasional evenings and weekends each month.
• Current CPR certification (BLS) is required.
• Preferred experience as a leader within an interdisciplinary care team.
• Experience in a startup environment is advantageous, though not mandatory.
• Must have access to reliable transportation and be willing to adhere to all terms of the Nest Motor Vehicle Safety Policy.
