Nurse Practitioner

This is a fully remote position, open to applicants in United States.

📋 Description

• Perform initial telehealth consultations, which may include E/M or Annual Wellness Visits, to determine the medical necessity for care navigation services.

• Recognize high-risk health conditions and address unmet social needs.

• Record findings in Baba’s platform, ensuring precise coding, SDOH Z-codes, diagnoses, and risk factors are accurately documented.

• Create and approve personalized care plans that are in alignment with patients’ medical treatment strategies.

• Work collaboratively with care advocates, nurse care managers, and clinical operations to finalize care plans and ensure safe and compliant patient enrollment.

⛳️ Requirements

• Graduation from an accredited Nurse Practitioner program (mandatory).

• Possession of an active Nurse Practitioner or APRN license in good standing, with licensure across multiple states (mandatory).

• Minimum of 2 years of experience in primary care, telehealth, or care management (mandatory).

• Knowledge of Medicare programs, care navigation, incident-to requirements, and SDOH documentation.

• Competence in using EMRs, care management platforms, and telehealth workflows.

• Holding multiple active state Nurse Practitioner licenses.

• Legal authorization to work in the US.

• Bilingual skills are a plus.
