Nurse Practitioner

This is a fully remote position, open to applicants in United States.

📋 Description

• Conduct virtual assessments of patients via video consultations.

• Manage medication for patients.

• Create care plans and ensure patient follow-up.

• Collaborate with primary care physicians, specialists, managed care organizations, and community partners.

• Facilitate advance care planning and discuss goals of care with patients and their families.

• Assist with HCC coding, risk adjustment, and closing quality gaps related to HEDIS and STAR through documentation.

• Engage in a shared rotating after-hours triage component available 24/7.

⛳️ Requirements

• Active Nurse Practitioner license in Delaware.

• Must reside in Delaware or nearby states and hold a Delaware license.

• Board certification as a Family Nurse Practitioner (FNP) or Adult Gerontology Nurse Practitioner (AGNP) is mandatory; general NP certification is insufficient.

• At least 3 to 5 years of experience in independent Nurse Practitioner practice.

• Proven experience in primary care or management of chronic diseases over time.

• Extensive and active experience with telehealth.

• Ability to make clinical judgments based solely on video consultations without physical access, vital signs, or remote monitoring tools.

• Experience in value-based care or a solid understanding of its principles.

• Knowledge of HCC coding, HEDIS measures, and STAR ratings in everyday practice.

• Current DEA registration and full prescriptive authority.

• Preference for candidates with Medicare and Medicaid provider numbers.

• An entrepreneurial mindset and comfort in a startup environment.

• This position must be the candidate's primary focus.

• Candidates should not hold multiple active employer commitments that conflict with a Monday to Friday, 8:00 AM to 5:00 PM schedule.
