Nurse Practitioner – 100% Virtual

This is a fully remote position, open to applicants in New Jersey, +1 more state.

📋 Description

• Collaborate with physicians, other healthcare providers, patients, and their families to create comprehensive care plans.

• Deliver both clinical and non-clinical support to patients.

• Offer urgent healthcare services through telephone and tele-video for individuals utilizing home and community-based services.

• Design and execute clinical care plans for adult patients with chronic and complex health issues.

• Collect patient histories, conduct physical examinations, and perform necessary diagnostics.

• Formulate and implement treatment plans tailored to patients' goals and existing health conditions.

• Identify and bridge gaps in patient care.

• Address the physical and psychosocial needs of patients and their families with the support of an interdisciplinary team.

• Educate patients and their families regarding medication use, potential side effects, illness progression, nutrition, adherence, and crisis intervention.

• Coordinate care and resources among clinical team members and medical providers.

• Maintain accurate patient medical records and documentation.

• Engage in required continuing education activities.

• Prescribe medications within the limits of state prescribing authority.

• Work Monday to Friday from 8:00 a.m. to 5:00 p.m. EST, with rotating on-call responsibilities 2 to 3 days each month.

⛳️ Requirements

• Master of Science in Nursing (MS).

• Active national Nurse Practitioner (NP) certification.

• Valid, current, active, unrestricted Family or Adult Nurse Practitioner license in Ohio and/or New Jersey.

• Valid, current, active RN Compact License.

• A minimum of 2 years of experience in managing complex care cases.

• Familiarity with Electronic Medical Records (EMR).

• Possession of DEA registration or eligibility is preferred.

• Active Medicaid number in Ohio and/or New Jersey is highly preferred.

• Strong preference for experience managing complex care cases involving developmental disabilities and chronically ill patients.

• Must be available to work Monday to Friday from 8:00 a.m. to 5:00 p.m. EST, including rotating on-call coverage.

• Candidates should live within a reasonable commuting distance from an office unless an accommodation is provided.
