Nurse Navigator – FIND Program

This is a fully remote position, open to applicants in Michigan.

📋 Description

• Facilitates communication and coordinates care among the patient, primary and specialty care physicians, and hospital departments following standardized care pathways.

• Ensures the availability, accessibility, and promptness of services to enhance the quality of care, coordination, and patient experience.

• Provides assessment, education, coordination, and referral services to patients identified with incidental radiological findings requiring recommended follow-up.

• Collaborates effectively with other FIND/LCS staff members.

⛳️ Requirements

• Graduate from an accredited nursing program and holds a Bachelor of Science Degree in Nursing.

• A minimum of three (3) to five (5) years of progressive clinical nursing experience is required.

• Current licensure to practice as a Registered Nurse in the State of Michigan.

• Strong problem-solving skills and the ability to generate innovative solutions to challenges.

• Demonstrated proficiency in navigating various software applications.

• Preferred working knowledge of Radiology procedures, protocols, and processes.

• Excellent organizational skills.

• Must exhibit a personal presence marked by honesty, integrity, and compassion, with the ability to inspire and motivate others to uphold the philosophy, mission, vision, goals, and values of Trinity Health.
