This is a fully remote position, open to applicants in United States.

📋 Description

• Collaborates closely with the clinical team to facilitate case development and management, along with care and treatment plans, ensuring accurate documentation and the identification of pertinent medical data while providing clear and concise communication to members and both internal and external partners.

• Acts as the primary coordinator for ongoing primary and preventative care, episodic care, expert medical opinions, and additional service lines.

• Gathers a comprehensive health history by guiding members through a systematic and dynamic intake assessment process.

• Addresses prescription issues related to episodic care in accordance with established protocols and turnaround times.

• Keeps availability updated in Outlook for scheduling member visits and all follow-up appointments.

• Leverages the company’s proprietary database to connect members with Teladoc Health and external physicians and completes referrals as necessary.

• Engages in member health coaching and provides treatment decision support through printed educational materials and videos.

• Applies clinical judgment when reviewing complex medical issues to ensure the accuracy of clinical summaries, reports, and care plans.

• Offers clinical support and guidance to support staff concerning case-related inquiries.

• Ensures compliance with established processes and adherence to privacy legislation and regulations with all parties involved in service delivery.

• Takes responsibility for meeting service standards regarding the speed of case progression, overall quality, and member satisfaction.

• Assists with additional projects as required.


⛳️ Requirements

• A minimum of 5 years of recent experience in an acute care setting is preferred.

• A background in med-surg is preferred.

• Quality-focused with a commitment to delivering exceptional customer service.

• Possesses a strong clinical knowledge base.

• Excellent written and verbal communication abilities.

• An outstanding team player with strong interpersonal skills.

• Bilingual in Spanish is a plus.

• Strong organizational skills with the capacity to multitask effectively.

• Ability to work independently while recognizing when escalation is necessary.

• Proficient in technology and software, including Microsoft Word, Excel, and Outlook.

• Current Registered Nurse license; a Bachelor of Science in Nursing (BSN) is preferred.
