This is a fully remote position, open to applicants in Canada.
• Provide assistance to patients and healthcare providers by delivering information about Patient Support services.
• Aid patients and caregivers in performing assessments related to Patient Support services.
• Serve as a mediator between patients, families, and healthcare providers regarding medication access.
• Respond to general inquiries about program services, disease states, or product-related questions.
• Deliver ongoing support concerning disease states or product-related inquiries to promote patient adherence.
• Direct patients to their healthcare providers for clinical questions that extend beyond program materials.
• Record patient information and interactions in the electronic patient record system.
• Conduct regular file reviews and updates to accurately reflect the patient journey.
• Adhere to case-management protocols, including data collection, patient follow-up, and monitoring therapy disruptions or discontinuations.
• Collaborate with the reimbursement specialist and physician to obtain funding for patient therapies.
• Ensure that patient services are coordinated and tracked in accordance with KPIs, leading to expedited access to therapy.
• Participate in specialized training sessions for specific therapies organized by the pharmaceutical team.
• Attend pharmaceutical meetings, patient meetings, conferences, and engage with patient associations and advocacy groups as necessary.
• Coordinate with healthcare providers, pharmacies, and infusion/injection clinics for treatment logistics and issue resolution.
• Maintain clear communication with the manager, team, and client regarding the management of patient cases.
• Assist in the training of new Nurse Case Managers and provide feedback on protocols, databases, processes, and inter-departmental procedures.
• Report Adverse Events / Severe Adverse Events in accordance with program procedures.
• A Diploma in healthcare or a related field is required, with a preference for a Bachelor’s degree.
• Active registration as a Licensed Nurse (RN or RPN/LPN) from the College of Nurses in the province of employment is mandatory.
• A minimum of two (2) to five (5) years of experience in healthcare, pharmaceutical, insurance, or related fields is essential.
• Ideal candidates should possess a nursing background with experience in healthcare and case management within medical/hospital settings, insurance, or the biopharmaceutical industry.
• Fluency in French and/or English may be necessary, depending on the program and geographic location.
