Nurse Case Manager

This is a fully remote position, open to applicants in Tennessee.

📋 Description

• Assisting with utilization management tasks for more intricate and non-standard cases as necessary.

• Acting as a bridge between members, providers, and both internal and external customers to facilitate health care delivery and benefits programs.

• Managing highly complex cases identified through multiple channels to ensure the effective implementation of interventions and the efficient use of benefits.

• Executing the fundamental activities of case management: assessment, planning, implementation, coordination, monitoring, outcomes, and evaluation.

⛳️ Requirements

• Active Registered Nurse (RN) license in the state of Tennessee.

• A minimum of 3 years of clinical experience is required.

• At least 5 years of experience in the healthcare sector.

• A minimum of 2 years of experience in IDD for Select Community is mandatory.

• Proficiency in PC skills is required (Basic Microsoft Office and Email).
