This is a fully remote position, open to applicants in United States.
• Oversee the comprehensive care management of a designated patient panel utilizing nursing expertise and knowledge of value-based care operations.
• Build trusting and compassionate relationships with patients and their families, offering both clinical and emotional support.
• Act as an advocate for patients and function as a community liaison to aid in adherence to treatment plans.
• Evaluate the needs of patients and caregivers, incorporating social determinants of health, and develop patient-focused care plans.
• Coordinate interdisciplinary care to ensure continuity, manage the progression of kidney disease, and facilitate provider collaboration.
• Track the effectiveness of care management plans and adjust interventions as necessary.
• Adhere to evidence-based guidelines, workflow protocols, quality metrics, as well as regulatory and payer requirements.
• Work alongside physicians, community providers, APPs, and other clinical disciplines to develop integrated care plans.
• Identify cost-effective strategies that support the goals of value-based care and efficient resource use.
• Educate patients and caregivers about treatment options to empower informed decision-making in care.
• Facilitate smooth transitions of care and address obstacles between different care settings.
• Engage in clinical huddles and patient care conferences.
• Contribute to organizational process enhancements and optimize workflows and engagement strategies.
• Prepare reports and deliverables concerning program changes and developments.
• Collect and present reports on program effectiveness, patient outcomes, and satisfaction levels of patients and caregivers.
• Execute additional duties related to the role as assigned.
• Associate degree in nursing is required.
• A current RN License is mandatory; a Compact License is preferred.
• Experience in care management is essential.
• Certification as a Case Manager is preferred.
• Proficient in MS Office Suite, including Outlook and Teams, at an intermediate level.
• Ability to effectively work in a predominantly remote setting.
• Home internet connection must provide at least 25 Mbps download and 10 Mbps upload; hardwired cable, fiber, or DSL is recommended.
• Must operate from a dedicated workspace free from ongoing interruptions to protect member PHI/HIPAA data.
