This is a fully remote position, open to applicants in Connecticut.
• Conduct thorough telephonic and virtual clinical evaluations, which include reviewing and interpreting biometric, RPM, and IVR data with trend analysis.
• Create, implement, and modify personalized nursing care plans that align with patient objectives, evidence-based practices, and organizational protocols.
• Utilize Motivational Interviewing techniques to engage patients, promoting behavior change, adherence, and self-management.
• Identify clinical risks, monitor changes in patient conditions, and address urgent situations; escalate appropriately according to clinical guidelines.
• Collaborate with interdisciplinary care teams (both internal and external) to ensure coordinated care, referrals, and transitions throughout the continuum.
• Provide proactive interventions aimed at minimizing hospital admissions, emergency room visits, and overall healthcare costs.
• Accurately, promptly, and comprehensively document all patient interactions in the electronic health record (EHR).
• Engage in quality improvement initiatives, chart audits, and performance evaluations to enhance clinical outcomes and care delivery.
• Execute care management tasks in accordance with client-specific workflows, performance expectations, and service-level requirements.
• Ensure adherence to all clinical, regulatory, HIPAA, and organizational standards.
• Nursing degree from an accredited institution (ADN or BSN required; BSN preferred).
• At least 5 years of extensive clinical nursing experience.
• Minimum of 3 years of experience in one or more of the following areas: Care management or care coordination, Home health or transitions of care, Telehealth or virtual care, Chronic disease management (e.g., cardiac, pulmonary, endocrine).
• Strong computer skills, including EHR documentation, navigating multiple systems, and troubleshooting basic technical issues.
• Ability to work within assigned time zones and schedules based on client requirements.
• Active, unrestricted RN license Compact (NLC) license and/or California RN license required, based on assignment.
