Nurse Care Associate

This is a fully remote position, open to applicants in Ohio.

📋 Description

• Perform telephonic clinical outreach to patients participating in Remote Patient Monitoring (RPM) and chronic care programs.

• Assess biometric alerts and pinpoint out-of-range readings; escalate clinical issues as per established protocols.

• Deliver patient education and coaching to enhance adherence to care plans and lifestyle suggestions.

• Meticulously record patient interactions in EMR/RPM systems, adhering to clinical and regulatory requirements.

• Collaborate with clinical management, RN Care Managers, healthcare providers, and interdisciplinary care teams to ensure coordinated patient care.

• Assist in patient engagement initiatives aimed at boosting monitoring compliance and improving clinical outcomes.

⛳️ Requirements

• Current LPN/LVN license in good standing.

• Over 5 years of clinical experience, including at least 3 years in care coordination, telehealth, home care, or chronic disease management.

• Experience in supporting patients with conditions such as CHF, COPD, diabetes, or hypertension is preferred.

• Excellent telephonic communication, patient education, and motivational interviewing abilities.

• Familiarity with EMR/EHR systems and comfort in a technology-driven care setting.

• Capability to work independently within a structured remote clinical workflow.
