Mental Health Physician Assistant, NP

This is a fully remote position, open to applicants in New Jersey.

📋 Description

• Conduct telehealth follow-up consultations for psychiatric medication management in elderly patients.

• Monitor treatment outcomes and modify medications while considering age-related risks, polypharmacy, and existing medical conditions.

• Carry out cognitive screenings and assessments using tools such as MoCA, MMSE, or comparable instruments.

• Create initial care coordination and behavioral support strategies.

• Collaborate closely with nursing personnel who will provide ongoing monitoring and patient assistance.

• Communicate with the supervising psychiatrist as necessary.

• Ensure timely and precise documentation within the electronic medical record (EMR).

⛳️ Requirements

• Must be licensed and certified as a Physician Assistant or PMHNP in New Jersey (required).

• Experience in geriatric psychiatry, long-term care, or dementia care is highly preferred.

• Familiarity with cognitive assessment tools and behavioral management techniques.

• Comfortable providing care through telehealth platforms.

• Strong clinical judgment related to polypharmacy, fall risk, and medication safety in older adults.

• Ability to collaborate effectively within a team-based care framework.
