Mental Health Case Manager II – Therapist

This is a fully remote position, open to applicants in Missouri.

📋 Description

• Delivers case management and therapy services to patients and their families to improve psychosocial functioning.

• Executes duties and responsibilities in alignment with the organization's mission, values, and standards of service.

• Guarantees safety, confidentiality, regulatory adherence, and compliance with hospital and infection control protocols.

• Facilitates interdisciplinary care, advocates for patients and their families, and manages comprehensive and safe discharge planning.

• Conducts behavioral health case management, assessments, therapy, crisis intervention, and ongoing treatment planning, ensuring accurate documentation.

• Engages in effective communication with patients, families, and community partners while demonstrating strong clinical, organizational, and technological competencies.

⛳️ Requirements

• A Master's degree is required in Social Work, Counseling, or a related human services field.

• Must possess a Licensed Clinical Social Worker (LCSW), Licensed Master Social Worker (LMSW), or Licensed Professional Counselor (LPC) certification in Missouri.

• A minimum of 2-3 years of experience in Behavioral Health is preferred.

• The candidate must demonstrate effective oral and written communication skills, along with proficient computer capabilities.
