Medical Social Worker

This is a fully remote position, open to applicants in California.

📋 Description

• Oversee the daily functions of the social services team, including the implementation of policies, procedures, and collaboration with other clinical providers.

• Recruit, onboard, and mentor social workers, offering clinical supervision, observing their practice, and fostering their professional development.

• Supervise the assessment, planning, and evaluation processes for social services care.

• Ensure that clinical documentation adheres to state, federal, and ACHC guidelines.

• Collaborate with the Executive Director on the annual budget and resource allocation for the department.

• Enhance knowledge of community resources and facilitate connections for patients and referral sources to appropriate support services.

• Organize in-service training sessions and continuing education opportunities.

• Participate in quality improvement initiatives.

• Perform additional duties as necessary to support patient services.

⛳️ Requirements

• Master of Social Work (MSW) from an accredited institution.

• Valid California license as an LCSW.

• Minimum of 3 years of management experience, ideally in the home care sector.

• Strong understanding of healthcare regulations, ACHC standards, and business operations.

• Exceptional communication skills with the ability to establish trust with patients, families, staff, and the community.

• Self-motivated, organized, and adaptable; proactive with a strong ability to collaborate with others.
