This is a fully remote position, open to applicants in United States.
• The Medical Records Associate is tasked with collecting patient information from healthcare providers, categorizing conditions, treatments, and diagnoses, and establishing permanent medical records for reference and regulatory compliance.
• Oversee electronic health records to ensure adherence to regulations and facilitate data-driven decision-making within healthcare environments.
• Retrieve medical records from healthcare facilities, examining patient charts and records for precision, thoroughness, and conformity with established standards.
• Identify any discrepancies, missing data, or errors within the documentation.
• Confirm that records comply with pertinent regulations, accreditation standards, and applicable policies.
• Verify the accuracy of patient demographics, medical history, treatment details, and other significant information.
• Communicate with physicians or other healthcare professionals to clarify information or address discrepancies.
• Enter and update medical records for healthcare professionals and regulatory purposes.
• Ensure timely updates to medical records.
• Safeguard patient data and medical records, ensuring they remain confidential.
• Transcribe medical information from doctors’ written or verbal notes, including symptoms, potential diagnoses, prescribed medications, and tests.
• If applicable, transcribe patient data from eSource into sponsor EDC.
• Utilize a HIPAA-compliant electronic fax line to obtain medical records.
• Upload and manage the IntElligo database, including tracking progress and timeliness.
• Ensure all necessary forms and reports are present and accurately completed.
• Maintain the status of all patients' medical records within the database.
• Follow IRB-approved protocols.
• Uphold the privacy of patient information in alignment with HIPAA regulations.
• Perform additional duties as required or assigned.
• High School diploma or GED is required.
• At least one year of experience in a medical record reviewer role or as a medical assistant.
• A minimum of one year of experience transcribing medical information and executing data entry, including categorizing and coding both written and verbal patient information.
• Experience in clinical research or patient recruitment is highly preferred.
• Certification in health information management (e.g., RHIT, RHIA) is preferred.
• Strong attention to detail and precision is essential.
• Knowledge and capability to implement effective data encryption and protection practices.
• Proficiency in medical terminology is required.
• Exceptional written and verbal communication skills are necessary.
• Strong organizational skills with the capacity to manage multiple tasks within agreed-upon timeframes through effective prioritization in a fast-paced environment.
