This is a fully remote position, open to applicants in Arizona.
• Collaborate with the Market President to establish and oversee the entire market team throughout Arizona.
• Direct clinical teams within the market, including Nurse Practitioners and Physician Assistants.
• Participate in direct patient care with a select group of high-risk patients.
• Provide in-home care at patients' residences, community centers, dialysis units, or hospitals.
• Ensure timely appointments for new patients.
• Maintain thorough and accurate clinical documentation.
• Enhance team members’ problem-solving abilities to minimize unnecessary medical utilization.
• Facilitate ongoing education and assessment of market-specific Advanced Practice Provider (APP) competencies.
• Instruct on diagnostic skills, evidence-based treatment protocols, treatment capabilities, and population health management.
• Partner with the Market President to achieve KPI targets and overall P&L objectives.
• Improve APP skills through intensive coaching based on KPI performance.
• Manage the highest-need and highest-risk patients, transitioning them to APP panels when suitable.
• Guide discussions with health plan and network provider partnerships.
• Cultivate partner relationships to enhance growth, engagement, and clinical/financial outcomes.
• Assist in the creation and content development for board materials.
• Execute additional responsibilities as assigned.
• Must reside in Arizona.
• Graduate with an M.D. or D.O. degree.
• Board Certification in Family Medicine or Internal Medicine is mandatory.
• Fellowship training in Geriatrics is required.
• Additional professional degrees (MPH, MBA, JD) are appreciated but not necessary.
• 5-15 years of experience in outpatient or home-based practices; experience in value-based care is preferred.
• Significant experience in clinical leadership roles, mentoring and developing Nurse Practitioners, Physician Assistants, or Medical Doctors.
• Proficient in using KPIs and data to enhance cost, quality, and clinical outcomes for patients.
• Ability to navigate ambiguity; experience in a startup environment is advantageous.
