Medical Case Manager

This is a fully remote position, open to applicants in Minnesota.

📋 Description

• Conduct visits with individuals on the caseload and participate in team meetings.

• Participate in or complete meetings for needs assessments and the development of individual service plans (CSSP).

• Assist in the planning and development of new services.

• Identify residential, vocational, and other essential services based on each individual's requirements.

• Oversee service delivery, ensuring that each person's health and safety needs are fulfilled.

• Complete referrals and provide crisis intervention as necessary.

• Develop budgets for waivers.

• Maintain accurate case notes.

• Serve as an advocate for the individuals receiving our services.

⛳️ Requirements

• Must be a licensed Social Worker (Bachelor of Social Work required for licensure).

• Alternatively, must be a licensed Graduate Social Worker (Master's in Social Work required for licensure) as per the Minnesota Board of Social Work.

• Or possess a 4-year degree in Human Services, Psychology, Sociology, or related fields.

• Must successfully pass the Department of Human Services background check.

• Must successfully clear the Motor Vehicle Background Check and maintain an acceptable driving record according to Company Policy.
