This is a fully remote position, open to applicants in Alabama, +13 more states.
• Oversee a designated panel of 400-450 patients and perform necessary monthly outreach to ensure ongoing care continuity.
• Make 50-60 outbound calls daily to patients, offering support for care plans and health coaching tailored to their individual needs.
• Uphold call quality standards by ensuring consistent engagement and professional communication in every interaction with patients.
• Work collaboratively with clinical teams, healthcare providers, and caregivers to identify and meet both clinical and social needs.
• Assist in achieving care coordination objectives by adhering to monthly outreach schedules.
• Address gaps in patient care through services such as Chronic Care Management, Behavioral Health Integration, and Remote Physiological Monitoring.
• Champion patient needs by actively listening to their concerns, investigating issues, and conveying solutions to the patient's care network.
• Record all patient interactions in real time using the care coordination platform.
• Ensure HIPAA compliance in all communications and documentation related to patients.
• Must possess a nationally recognized medical assistant certification (CMA, RMA, CCMA, NCMA, CMAC, or an equivalent state-issued MA-C).
• OR Certified Pharmacy Technician (CPhT).
• At least 1 year of clinical experience in primary care, a hospital, or a specialized healthcare environment.
• Familiarity with Electronic Medical Records (EMRs) and strong computer navigation capabilities.
• Proficient in various technology platforms and capable of navigating complex systems.
• Must have a HIPAA-compliant home office setup with a minimum of 50 Mbps download and 5 Mbps upload internet speed.
• Must be legally authorized to work in the United States.
