This is a fully remote position, open to applicants in Florida.

📋 Description

• Serve as an essential member of the Heart Failure Program, collaborating closely with the heart failure clinical team to deliver outstanding patient care, prioritize patient satisfaction, and ensure that our patients are at the forefront of our efforts.

• Manage the enrollment, engagement, and coordination of patients within the Heart Failure Program and other value-based care management initiatives.

• Supervise the telehealth schedules for the remote team.

• Reach out to and coordinate with patients post-hospital discharge for transitional care management.

• Facilitate patient care in collaboration with the remote patient monitoring care team.

• Conduct patient intake via phone, including gathering information on symptoms, vital signs, questionnaires, surveys, and social determinants of health (SDOH) needs.

• Deliver exceptional patient-centered care by ensuring that all patient needs are adequately addressed.

• Maintain and manage a patient cohort that necessitates regular follow-up calls, care management, and coordination.

• Identify and escalate patient situations that require attention from the appropriate care team provider.

• Keep patient records updated and confidential as per regulations.

• Ensure that medical records and lab results are available in patient charts before appointments and procure any missing records as needed.

• Adhere to prescription refill protocols.

• Collect and document updated insurance information from patients.

• Plan, organize, and provide educational resources and information to patients and caregivers.


⛳️ Requirements

• Certified Medical Assistant through the National Medical Assistant Certification exam (e.g., AAMA or NCCT) within one year of certification issuance.

• High school diploma or GED required.

• A minimum of 3 months’ experience in a medical office environment is preferred.

• Proficient computer skills are necessary as various programs, applications, and websites will be used.

• Familiarity with medical terminology and medication protocols.

• Experience in cardiology is advantageous, but not mandatory.

• Goal-oriented and focused on finding solutions.

• Strong organizational and interpersonal skills are essential.

• Basic understanding of CPT & ICD-10 coding practices.

• Ability to communicate effectively in English, both verbally and in writing, as well as in person and over the phone.
