Market Physician Executive – MPE

This is a fully remote position, open to applicants in New Jersey.

📋 Description

• Lead the multi-specialty polychronic care model in designated markets consisting of 5–10 practices.

• Develop and manage clinical strategies, policies, programs, processes, protocols, guidelines, and operations.

• Supervise daily clinical and business operations, including direct patient care, care management, social worker support, and pharmacy services.

• Review and authorize plans of care for APP, RN, social workers, and PharmD professionals.

• Review, approve, and co-sign APP encounters.

• Ensure compliance with evidence-based clinical pathways and the Monogram Model of Care.

• Drive clinical outcomes, intervention completion, utilization management, prescribing, therapy management, and HEDIS/gap closure.

• Lead daily high-risk and concurrent review rounds.

• Supervise frontline clinical and operational team members.

• Delegate operational tasks to the Market Manager, covering engagement, scheduling, administration, strategic implementations, and P&L management.

• Present market performance and outcomes to executive and senior leadership.

• Provide both direct and indirect patient care, encompassing disease diagnosis and treatment.

• Collaborate with patients, community physicians, facilities, and health plan partners.

• Conduct peer-to-peer reviews.

• Order labs and referrals, and take actions to improve outcomes and address care gaps.

• Guide population health management initiatives, focusing on high-risk patient interventions and reducing admissions/readmissions.

• Conduct post-hospital discharge visits and patient assessments.

• Collaborate with Medical Economics, Finance, and other stakeholders regarding utilization trends.

• Participate in Monogram on-call activities, committing to at least 7 days on call each quarter.

• Provide coverage for other MPEs during PTO or vacancies.


⛳️ Requirements

• Willingness and capability to obtain hospital privileges at designated facilities.

• Proven experience in applying evidence-based clinical criteria.

• Background in renal care and geriatrics.

• Strong management and communication abilities.

• Active, unrestricted state medical license required in all states within the market.

• Experience with high-need Medicare Advantage and managed Medicaid populations.

• Familiarity with NCQA, HEDIS, Medicaid, Medicare, quality improvement, medical utilization management, and risk adjustment.

• Current unrestricted state medical license with no sanctions from Medicaid or Medicare.

• Willingness to obtain licenses in multiple states.

• MD or DO degree from an accredited medical institution.

• Board certified or board eligible in an ACGME-accredited specialty, such as Nephrology, Internal Medicine, Family Practice, Emergency Medicine, Critical Care, Cardiology, Endocrinology, Hepatology, or Geriatrics.
