📋 Description

• Supervise care coordination, clinical escalations, case management, and client placement throughout the CCS network.

• Direct specialized care and clinical case management teams to align with strategic goals.

• Create and execute clinical strategies to enhance care path adherence, client outcomes, and provider involvement.

• Provide clinical supervision for high-risk cases and escalations.

• Manage comprehensive assessment, planning, coordination, and evaluation of client care.

• Establish protocols for case reviews, risk stratification, and multidisciplinary teamwork.

• Track case management metrics and outcomes, aiming to enhance service delivery and client satisfaction.

• Oversee Care Coordinator operations and ensure timely processing of clinical reports and authorizations.

• Manage client placement procedures, referral pathways, service availability, and equitable service access.

• Collaborate with provider partners to maximize capacity and lessen barriers to care.

• Analyze clinical trends and performance data to inform decision-making and strategic planning.

• Ensure adherence to regulatory standards and internal policies.

• Build and maintain relationships with provider partners and offer clinical consultation.

• Facilitate professional development and training for clinical staff.

• Lead staffing decisions, including recruitment, performance management, and team assignments.

• Implement strategies to prevent and manage clinical escalations and oversee complaints and grievances.

• Collaborate with Clinical Excellence and other departments on treatment plan guidelines.

• Represent CCS during regulatory inspections, stakeholder meetings, and cross-functional initiatives.

• Assist with care path adjustments and new modalities.

• Perform additional responsibilities as assigned by the Vice President of Network Operations.


⛳️ Requirements

• Over 8 years of experience in a home-health environment with supervisory roles.

• Required: Licensed Clinical Psychologist, LMFT, LPCC, or LCSW.

• Minimum of 5 years of progressive leadership experience in clinical operations, focusing on access and client care management.

• Experience working with individuals with autism spectrum disorders and related developmental disabilities.

• Demonstrated success in strategic planning, program development, and managing remote teams.

• Preferred experience in strategic planning and collaboration with executive and key operational groups.

• Strong communication skills in the workplace, both reading and writing.

• Ability to engage continuously with clients, staff, and others using visual and auditory capabilities.

• Proficient in using a computer, iPhone, and virtual meeting platforms such as Zoom and Teams.

• Familiarity with evidence-based methodologies including ABA, ESDM, TEACCH, PECS, and PRT.

• Training in ABA/Behavioral Health Treatment and knowledge of community resources and agencies supporting children.

• Experience in interpreting and implementing CCS policies, procedures, and regulations.

• High proficiency in Microsoft Office Suite, particularly Excel and PowerPoint.

• Strong analytical, interpersonal, verbal, and written communication abilities.

• Knowledge of I/DD communities and historically marginalized groups.

• Experience in working with and communicating effectively with families.

• Sensitivity to ethnically, linguistically, culturally, and economically diverse populations.

• Ability to multitask and manage daily responsibilities autonomously.

• Capacity to engage with individuals at all organizational tiers.

• Ability to meet deadlines while exercising judgment, flexibility, patience, and discretion with confidential information.

• Commitment to self-reflection, feedback, and maintaining wellbeing-oriented boundaries.

• Dedication to a remote schedule that aligns with PST business hours.

• Capability to obtain and maintain criminal record clearance through the DOJ.

• Ability to secure and uphold clearance through the Office of Inspector General.

• Attendance at required training sessions is mandatory.
