This is a fully remote position, open to applicants in United States.
• Lead the in-home multi-specialty polychronic care model within a designated region, supporting all markets.
• Contribute to the development of clinical strategies, policies, programs, processes, protocols, guidelines, and operations.
• Oversee daily clinical and business operations, which include direct patient care, care management, social worker assistance, and pharmacy services.
• Review and approve care plans for APP, RN, SW, and PharmD personnel.
• Review, approve, and co-sign APP encounters.
• Ensure compliance with Monogram Health's clinical guidelines and evidence-based pathways.
• Maintain prompt documentation of clinical activities, interventions, and tasks using Salesforce and Athena.
• Account for clinical outcomes, overall cost of care, and Medical Loss Ratio.
• Lead daily high-risk and concurrent review rounds.
• Directly supervise frontline clinical and operational team members.
• Delegate operational duties to the Market Manager, which include patient engagement, scheduling, administration, strategic implementations, and P&L management.
• Assess and present market performance and outcomes to executive and senior leadership.
• Provide both direct and indirect patient care, encompassing diagnosis and treatment.
• Engage patients in discussions regarding treatment plans and evidence-based care pathways.
• Collaborate with community physicians, facilities, and health plan partners in peer-to-peer discussions and treatment decisions.
• Order labs and referrals, and take necessary actions to enhance outcomes and address care gaps.
• Guide Population Health Management initiatives, including interventions for high-risk patients and strategies for reducing admissions/readmissions.
• Collaborate with Medical Economics, Finance, and other stakeholders to analyze utilization trends.
• Present population health strategies during clinical and operational meetings.
• Conduct patient reviews focusing on high utilizers, high-risk patients, high-opportunity patients, and specific cohorts.
• Participate in Monogram on-call activities, committing to a minimum of 7 days on call per quarter.
• Provide coverage for other MPEs during their PTO or vacancies.
• Willingness and ability to obtain hospital privileges at necessary facilities.
• Proven experience in applying evidence-based clinical criteria.
• Experience in renal care and geriatrics is essential.
• Strong management and communication abilities are required.
• An active, unrestricted state medical license is required for each state within the market.
• Experience with high-need Medicare Advantage and managed Medicaid populations is preferred.
• Familiarity with NCQA, HEDIS, Medicaid, Medicare, quality improvement, medical utilization management, and risk adjustment is beneficial.
• Current unrestricted state medical license, with no Medicaid or Medicare sanctions.
• Willingness to obtain licenses in multiple states.
• MD or DO degree from an accredited medical institution is required.
• Board certification or board eligibility in an ACGME-approved specialty such as Nephrology, Internal Medicine, Family Practice, Emergency Medicine, Critical Care, Cardiology, Endocrinology, Hepatology, or Geriatrics.
• Occasional in-home patient treatment visits and some domestic travel will be necessary.
