This is a fully remote position, open to applicants in Tennessee.
• The Market Physician Executive (MPE) is a mission-driven physician leader committed to enhancing the well-being, quality of life, and health outcomes for patients.
• The MPE will direct our in-home multi-specialty polychronic care model within a designated market.
• Each market consists of 5-10 practices managed by local advanced practice providers (APP), registered nurses (RN), licensed clinical social workers (LCSW), and pharmacists (PharmD).
• The MPE will work alongside Monogram Health’s Multi-Specialty Platform to utilize employed specialists for providing in-home specialty care.
• Monogram Health employs a validated risk-based model to promote health equity and equality, utilizing proprietary next-generation AI algorithms to determine the suitable level of care.
• In alignment with the quadruple AIM, the MPE will aim to enhance patient experience, population health outcomes, provider satisfaction, and reduce costs.
• The primary objective of each MPE is to achieve outstanding outcomes through disease detection, evidence-based clinical pathways, and effective disease treatment.
• Reporting to the Region President, the Market Physician Executive (MPE) serves as a key clinical leader at Monogram Health, contributing to the formulation and management of clinical strategies, policies, programs, processes, protocols, guidelines, and operations that enhance patient health outcomes within the market.
• The MPE supervises daily clinical and business operations by delivering direct patient care, care management services, social worker support, and pharmacy services within the market.
• Supporting advanced practice providers, the MPE is expected to evaluate and approve care plans while directing treatment plans for patients.
• The MPE will collaborate with community physicians, facilities, and partners in peer-to-peer and direct patient care decision-making.
• Overall responsibility for reducing total cost of care and Medical Loss Ratio.
• Accountable for clinical outcomes, including but not limited to clinical interventions closure, inpatient/outpatient utilization, pharmacological prescribing and therapy management, multi-specialty platform, and HEDIS/Gap Closure.
• Actively lead daily high-risk and concurrent review rounds.
• Provide direct supervision to front-line clinical and operations team members.
• Oversee and delegate operational responsibilities to the Market Manager.
• Regularly assess and present market performance and outcomes to Executive and Senior Leaders.
• Must be willing and able to secure hospital privileges at necessary facilities.
• Proven experience in applying evidence-based clinical criteria.
• Experience in renal care and geriatrics is required.
• Strong management and communication skills are essential.
• An active, unrestricted state medical license is required in each state within the market.
• Experience with high-need Medicare Advantage and managed Medicaid populations.
• Familiarity with NCQA, HEDIS, Medicaid, Medicare, quality improvement, medical utilization management, and risk adjustment.
• Current state medical license must be free of restrictions and sanctions from Medicaid or Medicare.
• Willingness to obtain licensure in multiple states.
• An MD (Medical Doctor) or DO degree from an accredited medical school is necessary.
• BC or BE in an ACGME-approved specialty such as Nephrology, Internal Medicine, Family Practice, Emergency Medicine, Critical Care, Cardiology, Endocrinology, Hepatology, or Geriatrics.
