This is a fully remote position, open to applicants in United States.
• Guide, mentor, and nurture a team of RN Case Managers, ensuring adherence to organizational policies and regulatory standards.
• Supervise daily outpatient case management operations, encompassing referrals, transitions of care, and care coordination.
• Maintain readiness for audits by coordinating chart reviews and overseeing case management processes.
• Educate and onboard new staff while providing continuous coaching and performance evaluations.
• Allocate and prioritize team tasks to guarantee efficiency and high-quality outcomes.
• Collaborate across departments with Medical Directors, Quality, and Compliance leadership.
• Handle escalated cases and perform case management duties as necessary.
• Ensure accurate and compliant documentation for all team activities.
• Analyze and present clinical and program data to leadership to foster continuous improvement.
• Assist in the creation and execution of protocols, workflows, and best practices.
• Oversee a team of RN Case Managers and coordinators.
• Drive hiring, onboarding, and training initiatives.
• Deliver performance management, coaching, and development.
• A minimum of 5 years of RN Case Management experience (or an equivalent mix of education and experience).
• At least 1 year of leadership or supervisory experience in a healthcare environment.
• An Associate’s or Bachelor’s degree in Nursing is required.
• An active, unrestricted Registered Nurse (RN) license is mandatory.
• CCM certification is preferred.
• Comprehensive knowledge of Medicare Managed Care Plans.
• Demonstrated leadership and team management capabilities.
• Proficient in managing projects and initiatives aimed at enhancing performance.
• Strong skills in organizing, prioritizing, and delegating tasks.
• Exceptional communication and collaboration skills within diverse teams.
• Strong analytical, problem-solving, and decision-making skills.
