This is a fully remote position, open to applicants in Illinois.
β’ The Manager of Care Management is responsible for overseeing the assessment and evaluation of members' needs to ensure they achieve and maintain optimal wellness.
β’ Facilitate interactions and guide members and their families towards appropriate resources for care and wellbeing.
β’ Monitor program performance and leverage data to assess impact, identify gaps, and foster continuous improvement.
β’ Collaborate with internal teams and external partners to guarantee seamless integration of non-clinical support staff into care planning and service delivery.
β’ Develop and uphold policies and procedures that ensure consistent, high-quality service delivery throughout the care system, aligning with the organization's mission to promote health equity and reduce disparities.
β’ Assist in addressing individual member challenges related to housing, food insecurity, transportation, and other social needs.
β’ Represent the care management program in collaborative initiatives, advisory groups, and community forums.
β’ Actively participate as a member of the management team in advancing Humana's mission for strategic growth and development.
β’ Must be a resident of Illinois.
β’ A minimum of an Associate's Degree is required.
β’ Current Registered Nurse (RN) license or Social Work (SW) license is necessary.
β’ At least 5 years of professional experience is required.
β’ A minimum of 2 years of management or supervisory experience is needed.
β’ Proficiency in analyzing and interpreting data trends is essential.
β’ Experience in progressive operational leadership is required.
β’ Strong communication, analytical, problem-solving, and teamwork skills are a must.
β’ Knowledge of Medicaid/Medicare, along with long-term care guidelines, benefits, and policies is necessary.
β’ Proficiency in Microsoft Windows, Outlook, Excel, Word, and other MIS software applications is expected.
β’ A solid understanding of care management models and the role of extenders in meeting social needs is essential.
β’ Proven ability to lead cross-functional initiatives and collaborate effectively with external partners.
β’ Capability to work independently as well as part of a team is required.
