This is a fully remote position, open to applicants in Illinois.
• The Manager of Care Management is responsible for overseeing the assessment and evaluation of members' needs to ensure they achieve and maintain optimal wellness.
• Facilitate interactions and guide members and their families towards appropriate resources for care and wellbeing.
• Monitor program performance and leverage data to assess impact, identify gaps, and foster continuous improvement.
• Collaborate with internal teams and external partners to guarantee seamless integration of non-clinical support staff into care planning and service delivery.
• Develop and uphold policies and procedures that ensure consistent, high-quality service delivery throughout the care system, aligning with the organization's mission to promote health equity and reduce disparities.
• Assist in addressing individual member challenges related to housing, food insecurity, transportation, and other social needs.
• Represent the care management program in collaborative initiatives, advisory groups, and community forums.
• Actively participate as a member of the management team in advancing Humana's mission for strategic growth and development.
• Must be a resident of Illinois.
• A minimum of an Associate's Degree is required.
• Current Registered Nurse (RN) license or Social Work (SW) license is necessary.
• At least 5 years of professional experience is required.
• A minimum of 2 years of management or supervisory experience is needed.
• Proficiency in analyzing and interpreting data trends is essential.
• Experience in progressive operational leadership is required.
• Strong communication, analytical, problem-solving, and teamwork skills are a must.
• Knowledge of Medicaid/Medicare, along with long-term care guidelines, benefits, and policies is necessary.
• Proficiency in Microsoft Windows, Outlook, Excel, Word, and other MIS software applications is expected.
• A solid understanding of care management models and the role of extenders in meeting social needs is essential.
• Proven ability to lead cross-functional initiatives and collaborate effectively with external partners.
• Capability to work independently as well as part of a team is required.
