LVN Home Health, Full Time

This is a fully remote position, open to applicants in Texas.

📋 Description

• Deliver proficient nursing care to patients in their residences under the guidance of a Registered Nurse.

• Execute routine skilled nursing visits in accordance with physician directives and tailored care plans.

• Assess patient conditions, monitor vital signs, and track progress against set objectives.

• Administer wound care, medication education, disease management, and other skilled nursing tasks within the LVN scope of practice.

• Promptly report any changes in patient condition to the supervising RN and clinical management team.

• Collaborate with physicians, therapists, and other healthcare professionals to ensure coordinated patient care.

• Maintain accurate and timely electronic clinical documentation utilizing AI-supported EHR tools.

• Uphold a productive weekly visit schedule while providing high-quality patient care.

• Deliver care within patients' homes across a designated geographic area.

⛳️ Requirements

• Possess a current and unrestricted Texas LVN license.

• Prior experience in home health is highly preferred.

• Emphasis on wound care is advantageous.

• Familiarity with electronic medical records; experience with WellSky/Kinnser is a plus.

• Strong skills in clinical assessment, communication, and time management.

• Capability to work independently and manage a steady patient caseload.

• Access to reliable transportation.

• Valid driver's license required.

• Necessary auto insurance coverage.

• Commitment to compassionate, patient-centered care.

• Must reside in the greater San Antonio area.

• Ability to complete approximately 35–40 visits weekly.

• Willingness to operate within a designated geographic territory.
