LTSS Service Coordinator – RN Clinician

This is a fully remote position, open to applicants in United States.

📋 Description

• Oversee members' cases within the boundaries of licensure, providing guidance and support to non-RN clinicians.

• Perform telephonic or in-person clinical evaluations to assess and understand members' needs, encompassing physical health, behavioral health, social services, and long-term services and supports.

• Recognize members at elevated risk for complications and coordinate care in conjunction with the health care team.

• Create, monitor, assess, and adjust personalized care plans to enhance member health throughout the care continuum.

• Set both short- and long-term objectives in partnership with members, caregivers, families, and physicians.

• Identify members who might benefit from alternative levels of care or waiver programs.

• Aid in the execution of member care plans by facilitating authorizations/referrals for necessary services.

• Collaborate with Medical Directors, Physician Advisors, and Inter-Disciplinary Teams on care management treatment strategies.

• Supervise and/or direct the activities of non-RN clinicians, ensuring appropriate task assignments and regular performance evaluations.

⛳️ Requirements

• Registered Nurse (RN) with at least 3 years of experience working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator, Case Management, or similar position.

• Current, unrestricted RN license in Indiana.

• MA/MS in Health/Nursing is preferred.

• State-specific certification may be necessary based on state law and/or contract requirements.

• Training, expertise, and experience in delivering case management and care coordination services for individuals, including specialized populations such as older adults and/or individuals with physical or developmental disabilities and/or those identified as having a serious mental illness (SMI).

• Excellent communication, organizational, and problem-solving abilities.

• Capability to work efficiently in a collaborative team environment.
